This is particularly important in the elderly, in whom clinical outcomes that may not be directly related to a drug’s therapeutic indication can have a major economic impact.

Cost-effectiveness analysis is still subject to bias or sensitivity to initial assumptions. Nonetheless, some consensus is emerging concerning methodologie standards in this field, and the effects of biases and assumptions often can be discerned if they are presented openly and modeled appropriately. However, the rapidly increasing use of cost-effectiveness analysis will need to be matched by a rapid increase in the sophistication of both practitioners and consumers of such analyses if this approach is to retain legitimacy in both the marketplace and academic circles. Even when such analyses are performed impeccably, their conclusions must be seen as relating to populations of patients, rather than constraining prescribing for individual patients regardless of idiosyncratic differences. Given the very high benefits, risks, and costs of medication use in the elderly, they could serve as an appropriate subgroup in which to develop a sound approach to these issues.
