Community education is a critical part of community health nursing, as is the establishment of community partnerships. The purpose of this requirement is to assist the student to develop competency in both of these areas.

If agreeable, set up a timeframe to assess the group and plan an educational intervention. Work on the this assignment while you are meeting with the group over a period of time (days to weeks). This assessment is your fieldwork with your target population. You can meet with the member individually, in small groups, or all together. You can meet via phone call, text, email, social media, or in-person using social distancing and safety precautions.

For this assignment you will be completing the Target Population Assessment template (see forms in course). You should be addressing all the components in this template. You should be using the information you gathered from your target population along with the research you have gleaned from the literature on the topic you have chosen to provide a strong defense for why this topic is important for the target population. You should be placing in-text citations for your sources and a full reference list at the end of the template. You should be using APA formatting to complete this assignment, which means paraphrasing, no use of first person, correct grammar and scholarly sources (peer-reviewed) dated no less than 5 years. Please do not use Wikipedia, UpToDate, or sources that are designed for the general public. The assignment rubric is built into the template.
 
Work with your target population that you have chosen to provide a health promotion plan. Include in this discussion, a brief synthesis of interviews with group members and at least three significant findings you have identified regarding your target group in your community.

  1. Describe the Target Population you are working with.
  2. Identify any factors that place your target group at risk for chronic diseases. Please provide a description of the factors and why you think these are relevant. You might want to choose this topic for your health promotion project. Is there a topic your group members are interested in learning more about?
  3. Choose and explain two goals outlined in Healthy People 2020/2030 that relate to your target group. In reference to those goals, identify any obstacles you feel your target group might have to overcome in order to achieve these goals. What is different between Healthy People 2020 and 2030 goals?
  4. Describe any primary or secondary disease prevention strategies that would benefit your target group. Provide information on how you anticipate these preventative measures would work and what they would prevent.
  5. Review the county web site to see if there are any initiatives related to this topic. On the Internet, go to your county health department web site and there should be an area labeled Programs & Services, or something like that.
  6. Review the popular and scholarly literature on this topic. Does the popular literature reflect the scholarly sources? In other words, is the popular literature true and factual, or hearsay, rumors, just people’s ideas? Did you find any myths reported about your topic (e.g. Lost 50 pounds in 5 days with this drink!)?
