While the focus of this course is nursing theory, some nurses say that theory has no role in clinical practice. What are your thoughts? Does nursing theory have a role in clinical practice? How would you use nursing theory to improve or evaluate the quality of clinical practice? Be sure to include an example that demonstrates your thoughts. Some nurses say that theory has no role in advance clinical practice. Nursing is a little different from most health-care professions in that it is theory based.

Our first nursing theorist was Florence Nightingale. Nightingale used theory to explain what is and is not nursing, and this is a tradition that continues in nursing to this day (McEwen & Wills, 2014).

Nursing theory facilitates the development of nursing knowledge and provides principles to support nursing practice. Theory outlines practice and provides a way for communicating key ideas regarding the core of nursing practice (Walker & Avant, 2011). Theory is used in all facets of nursing care and assists the nurse in organizing, understanding and analyzing patient data.

Essentially, theory provides a systematic, reliable way of thinking about nursing care to guide the decision-making process. Theory-based, clinical practice occurs when nurses intentionally structure their nursing practice around a certain theory to guide them in their patient care (Matney, Avant, & Staggers, 2015).

Yes, I believe that nursing theory has a role in clinical practice. In fact, I would venture to say that it plays a foundational role in clinical practices. A nursing professional practice model (PPM) is, “The guiding theoretical and conceptual model that frames the foundation for nursing professional practice” (Cordo, 2017, p. 325). Council members within the PPM determined three nursing theories that they felt would coordinate well with the organizational diverse customs, objective, insight, and principles (Cordo, 2017).  The chosen theories were Katharine Kolcaba’s Comfort Theory, Madeleine Leininger’s Transcultural Nursing Theory, and Jean Watson’s Caring Theory (Cordo, 2017). This clearly shows that nursing theory has a function and goal within the clinical practice.

For example, Kolcaba’s Comfort Theory is exemplary of a versatile, well-rounded PPM with the ever emphasized holistic approach that coordinates with the principles, objectives, and goals within the institution (Cordo, 2017). In conjunction with this, the fundamental and central focus in pediatrics is individualized, holistic care, synergistic goal setting, and disease prevention (Cordo, 2017). According to Kolcaba’s theory, when comfort is attained, the child and family or loved ones are able to engage in endeavors needed to obtain or maintain health and wellness through therapeutic rapport (Cordo, 2017).

An additional example is The Normalization Process Theory (NPT) which can be enforced for understanding the mechanisms of invoking clinical interventions (May, 2014).  NPT delineates the implementation, implanting and synthesis of a clinical practice protocol- or any other  manner of clinical or institutional intervention- as a social process that transpires when contributors intentionally commence and pursue to maintain series of actions that bring into operation (May, 2014). NPT also aids the inquiry of nursing work by converging on the techniques that clinical nursing practice is made comprehensible and useful, how it contributes to an array of mutual commitments, how the improvements are executed and reviewed, and how they are evaluated and reconstructed (May, 2014).
